Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  A.) Paramount Roofing, Inc., 5130 Peach Street, Erie, PA 16509, on or about December 10, 2020 and times prior thereto:  Employee engaged in roofing activities on a commercial low-sloped foot were exposed to a fall hazard of 20.2-feet on the west side of the structure.  Employees were not using an acceptable means or method of fall protection.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Paramount Roofing, Inc., 5130 Peach Street, Erie, PA 16509, roof, on or about December 10, 2020 and times prior thereto: Employees were stepping up onto a higher roof level when traveling from the ladder access to the work area. The break in elevation was 2.4-feet.  D.) Paramount Roofing, Inc., 5130 Peach Street, Erie, PA 16509, roof, on or about December 10, 2020 and times prior thereto: Employees were stepping down to a lower roof level when traveling from the work area to the ladder egress. The break in elevation was 2.4-feet.  E.) Paramount Roofing, Inc., 5130 Peach Street, Erie, PA 16509, roof, on or about December 10, 2020 and times prior thereto: Employees were stepping over a interior parapet wall when traveling from the ladder access to the work area. The parapet wall height was 24-3/8-inches on the south side of the wall and 25-3/8-inches on the north side of the wall.  F.) The employer's hazard communication program was deficient in that it did not include the following information:      (1) A designated person(s) responsible for workplace labeling,     (2) Procedures to review and update label information when necessary,     (3) A designated person(s) responsible for obtaining/maintaining the SDSs,     (4) Procedure to follow if the SDS is not received at the time of the first shipment,     (5) Procedure to follow if it is suspected that the SDS is not appropriate,     (6) Procedure to follow to determine if the SDS is current,     (7) A designated person(s) responsible for conducting the hazard communication training,     (8) Information on how the hazard communication training is to be conducted, and      (9) Procedure to train new employees at the time of their initial assignment and to train employees when a new hazard is introduced into the workplace.
