29 CFR  1910.146(f)(4): The entry permit that documented compliance and authorized entry to a permit space did not identify the authorized entrants within the permit space by name or by such other means (for example, through the use of rosters or tracking):   (a) On 2/12/2021 - Fuel House - Upper (a.k.a. Tee Pee), employees entering a confined space were not identified nor listed on the entry permit and exposed to hazardous energy and/or engulfment hazards from accumulated sawdust when they accessed the silo  to grease fittings and change out a belt.  (b) On 2/18/2021 - Fuel House - Upper (a.k.a. Tee Pee), employees entering a confined space were not identified nor listed on the entry permit and  exposed to hazardous energy and/or engulfment hazards from accumulated sawdust when they accessed the silo to work on the agitator.  (c) On 2/25/2021 - Fuel House - Upper (a.k.a. Tee Pee), employees entering a confined space were not  identified nor listed on the entry permit and exposed to hazardous energy and/or engulfment hazards from accumulated sawdust when they accessed the silo to replace a broken belt.  (d) On 3/5/2021 - Fuel House - Upper (a.k.a. Tee Pee), employees entering a confined space  were not identified nor listed on the entry permit and  exposed to hazardous energy and/or engulfment hazards from accumulated sawdust when they accessed the silo to replace a drive chain.  (e) On 3/11/2021 - Fuel House - Upper (a.k.a. Tee Pee), employees entering a confined space were not identified nor listed on the entry permit and  exposed to hazardous energy and/or engulfment hazards from accumulated sawdust when they accessed the silo to put on a belt.

Tee Pee), employees entering the silo were exposed to hazardous energy and engulfment hazards due to the accumulation of sawdust and adequate measures such as, but not limited to the employer not identifying the Rescue and Emergency Services or how to contact the Rescue and Emergency Services for entries dated between 1/6/2021 to 5/4/2021.  Lewis Brothers Lumber Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.146(d)(9), which was contained in OSHA inspection number 1434934, citation number 1, item number 3 and was affirmed as a final order on 4/15/2020, with respect to a workplace located at 1031 13th Street NW, Aliceville, AL 35442.
