Tee Pee), employees entering a confined space were not identified nor listed on the entry permit and exposed to hazardous energy and/or engulfment hazards from accumulated sawdust when they accessed the silo to grease fittings and change out a belt. (b) On 2/18/2021 - Fuel House - Upper (a.k.a. Tee Pee), employees entering a confined space were not identified nor listed on the entry permit and exposed to hazardous energy and/or engulfment hazards from accumulated sawdust when they accessed the silo to work on the agitator. (c) On 2/25/2021 - Fuel House - Upper (a.k.a. Tee Pee), employees entering a confined space were not identified nor listed on the entry permit and exposed to hazardous energy and/or engulfment hazards from accumulated sawdust when they accessed the silo to replace a broken belt.
