29 CFR 1910.95(g)(1):The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: Employees were exposed to excessive noise levels and risk of hearing loss. Employees performing bone sawing and tumbling operations were exposed to an 8 hour Time Weighted Average of 88.4 DbA. The employer had not provided exposed employees with audiometric testing. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

Employees working in the bone room performing tasks related to bone cleaning and sawing were exposed to a Time Weighted Average of 88.6 DbA. The employer had not provided employees exposed to excessive noise levels with a baseline audiogram within 6 months of an employees first exposure at or above the action level. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
