29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Worksite, 125 Spagnoli Road, Melville, New York 11747:  On or about December 09, 2020, the employer did not provide documented energy control procedures for machines such as but not limited to, the Kidder-Stacy Flexographic Printing Press Model No. 660/Serial No. 971-43.   Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR  1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a) Worksite, 125 Spagnoli Rd, Melville, New York 11747,   On or about December 09, 2020, an employee installing printing cylinders to a Kidder-Stacy Flexographic Printing Press was not trained on the purpose and use of the energy control procedures.  Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR  1910.1200(e)(1):Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   a) Worksite, 125 Spagnoli Rd, Melville, New York 11747,   On or about December 14, 2020, employees were authorized to use chemicals in the workplace such as, but not limited to, Poly-Pak 80/20 solvent, Calumet 150-190 solvent, n-Propanol lubricant, n-Propanol acetate solvent, Blend 80/20 solvent, Dowanol PM Glycol Ether STD, and UN1210 Printing Ink. A written hazard communication program was not in place.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
