(a) At the work site, 125 Spagnoli Rd, Melville, NY; Press Shop, where press number 4 is used, the employer did not provide the employee with an annual audiogram. The employee was exposed to an 8-hour time-weighted average of 89.7 decibels. on or about 3/18/21 Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
