Building Integrated Services, LLC., Compressor Room substation area located at 2424 West 23rd Street, Erie, PA, on or about December 12th, 2020 and times prior thereto: An employee performed preventative maintenance work operations on or around a Load-Break Interrupter Switch (Type: SE100S; Serial # 0213X0300G3) associated with the Compressor Room substation unit (12,470 volts). All energized components associated with the Load-Break Interrupter Switch were not completely de-energized prior to performing work operations. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

The Load-Break Interrupter Switch associated with the Compressor Room substation unit was not completely de-energized prior to preventative maintenance work operations in the and around the Load-Break Interrupter Switch area.
