29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million (ppm): On or about December 31, 2025, at a work location located at the Judson Senior Living Community South Franklin Circle, 16600 Warren Court Chagrin Falls, OH 44023. The employer did not ensure that an HVAC Technician performing a service call (troubleshooting) in the boiler room of building #2 was protected from over exposure to carbon monoxide at an 8-hour time-weighted average (TWA) exposure was 608.6 ppm CO ( SAE = 0.103), which is 12.17 times the OSHA PEL of 50 ppm CO.

Employees did not utilize Carbon Monoxide detection equipment (initial testing and or continuous monitoring device) to ensure that boiler room #2 was free of the presence of carbon monoxide (asphyxiant) in the work area.
