29 CFR 1910.147(c)(5)(ii)(C)(1): The employer did not ensure lockout devices were substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools: U.S. Postal Service - LAX ISC: The (Loose mail system) LMS Machine which loads mail to the APBS #3 machine was down for maintenance. Lock out/tag out (LOTO) was performed on the LMS and work had been in the process of repairing a section of damaged conveyor belt. The key to the LOTO device had been left in the lock during the repair which would potentially allow others to remove the LOTO device during maintenance services. Employees were exposed to unintentional/unanticipated equipment start up potentially causing electric shock and/or caught in-between hazard(s).

29 CFR 1910.219(c)(4)(i):Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: Basement, B-1 Conveyor: The employer did not ensure the cylinder shaft at the far end of the conveyor measured at 6.5 inches being more then one half the diameter of the 3.25 inch shaft had a fixed guard. This condition exposed employees to lacerations, amputations, employee(s) pulled into moving parts causing trips and falls and/or rotating moving part hazard(s).

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): Basement, B-1 Conveyor: The employer did not ensure the vertical 3/4 inch belt on the drive motor approximately 2 to 3 feet from the floor was equipped with a fixed guard. This condition exposed employees to the transverse motion movement of the belt and pulley which created a caught in a pinch point with potential for amputations and/or hazards of rotating parts.
