29 CFR  1910.147(c)(5)(ii)(C)(1): The employer did not ensure lockout devices were substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools:    U.S. Postal Service - LAX ISC: The (Loose mail system)  LMS Machine which loads mail to the APBS #3 machine was down for maintenance. Lock out/tag out (LOTO) was performed on the LMS and work had been in the process of repairing a section of damaged conveyor belt. The key to the LOTO device had been left in the lock during the repair which would potentially allow others to remove the LOTO device during maintenance services. Employees were exposed to unintentional/unanticipated equipment start up potentially causing electric shock and/or caught in-between hazard(s).
