29 CFR 1910.141(a)(5):Vermin control. Every enclosed workplace shall be so constructed, equipped, and maintained, so far as reasonably practicable, as to prevent the entrance or harborage of rodents, insects, and other vermin. A continuing and effective extermination program shall be instituted where their presence is detected. On or about December 18, 2020, the employer did not ensure that the workplace was kept free of rodents and vermin.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or before December 17, 2020, maintenance overhead door was not guarded to prevent body parts in the danger zone during the doors operating cycle.
