29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: On or about December 21, 2020, two workers were performing welding tasks in two separate aerial lifts that were being driven over temporary extension cords that were not protected from damage, exposing workers to hazards such as, but not limited to electrical shock, burns or fire.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about December 21, 2020, two workers were performing welding tasks in two separate aerial lifts up to approximately 12 feet high without being secured in the basket, exposing the workers to a fall hazard.
