29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees working in the Bulk Offload area where sodium hypochlorite was unloaded from truck trailer and stored in bulk tanks were exposed to off gassing chlorine from a chemical reaction of sodium hypochlorite and acid: On or about December 23, 2020 at Galloway Company facility located at 601 South Commercial Street, Neenah, WI 54956; the employer did not prevent a sanitizing chemical (Sani-I-King No. 451) containing sodium hypochlorite (~12.5%) from being unloaded via truck trailer to a bulk storage tank which contained an acid cleaner chemical (MPA No. 168) containing nitric acid and phosphoric acid. The mixture caused a release of chlorine gas into the Bulk Offload area which resulted in Galloway Company employee exposures to airborne chlorine gas and necessitated employees to evacuate the facility.

29 CFR 1904.29(b)(1): You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year: At the Galloway Company facility located at 601 South Commercial Street, Neenah, WI 54956; the employer did not clearly describe the objects/substances that directly injured or made persons sick in column (F) of the OSHA Form 300. For Case no. 20-3 which occurred on 12/13/2020, the employer described "Hernia" as the injury/illness with no description as to the object/substance that directly caused the injury/illness.
