29 CFR  1904.29(b)(3):Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301or equivalent) within seven (7) calendar days  of receiving information that a recordable injury or illness has occurred:  (a) U.S. Department of Navy DBA Naval Support Facility Indian Head located at 3838 Strauss Ave. Bldg. D28, Indian Head, MD 20640:  On or about December 23, 2020, the employer had not recorded a work-related illness of an employee on the OSHA's Form 301or its equivalent within seven calendar days of determining that a recordable illness had occurred.   (b) U.S. Department of Navy DBA Naval Support Facility Indian Head located at 3838 Strauss Ave. Bldg. D28, Indian Head, MD 20640:  On or about December 28, 2020, the employer had not recorded a work-related illness of an employee on the OSHA's Form 301or its equivalent within seven calendar days of determining that a recordable illness had occurred. 29 CFR  1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident    (a)  U.S. Department of Navy DBA Naval Support Facility Indian Head located at 3838 Strauss Ave. Bldg. D28, Indian Head, MD 20640:  On or about January 1, 2021,  an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within 8 hours. The employer reported the death to OSHA on January 4, 2021.  NO ABATEMENT CERTIFICATION OR DOCUMENTATION REQUIRED FOR THIS ITEM

29 CFR  1910.134(f)(2):Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:  (a.)  U.S. Department of Navy DBA Naval Support Facility Indian Head located at 3838 Strauss Ave. Bldg. D28, Indian Head, MD 20640:  On or about January 4, 2021,  the employer did not provide an annual  fit test to all employees required to wear respirators.  The employer required firemen to wear N95 filtering facepiece respirators to protect against SARS-CoV-2 when responding to calls involving patient encounters and Scott AV3000HT Full Face respirators in areas where the atmosphere is unknown or hazardous and/or inside of a confined space.
