Employee(s) were disconnected from personal fall arrest system(s) on the roof while fetching materials such as shingle bundles, exposing them to hazard of falling 17 feet from the roof eave to the ground below. Condition(s) were noted on or about 12/28/2020.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Employee(s) installed shingles atop a single-family house at 21 Arbutus Way in Monmouth Junction NJ 08852. An aluminum extension ladder leaned against the front (west-facing) side of roof was used for access to/from roof, which did not have side rails extended at least 3 feet above the eave edge; exposing roofers to the hazard of falling 17 feet from the top rung of the ladder. Condition(s) were noted on or about 12/28/2020.
