Penn Pallet, Inc. - On or about 12/19/20, and at times prior thereto, the foot pedal on a Whirlwind saw was not guarded  to prevent employees from accidentally activating the saw.  An employee sustained a fingertip avulsion when their hand was near the blade and they accidentally actuated the foot pedal.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Penn Pallet, Inc. - On or about 12/19/20, and at times prior thereto,  the Whirlwind saw was not guarded adequately and/or so designed and constructed as to prevent the employees from having any fingers in the saw blade area during operation of the saw blade.  While in operation/making the cut, the guard did not adequately cover approximately 5-inches of exposed saw blade.
