The saw blade did not have any guarding.

Reclaim area; on or about 02/01/2023 and at times prior thereto: Employees performing blade changes on the Smetco saw in the reclaim area do not apply their own personal lockout tagout device. Employees performing the blade change remove the covers on the Smetco saw while a maintenance employee applies his/her lockout tagout device.
