29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a.  The energy control procedure for the Paul Saw (Equipment # 249.00) did not identify all sources of potentially hazardous energy, such as but not limited to hydraulic and kinetic energies, exposing employees who were performing servicing and/or maintenance operations, to struck by and caught between or under hazards.  b.  The energy control procedure for the Paul Saw (Equipment # 249.00) did not describe the magnitudes of energies associated with the machine.  The saw operated on 480VAC, and the Lockout Procedure created by the employer did not describe that magnitude of energy.

29 CFR  1910.305(g)(1)(iv)(A): Unless specifically permitted, flexible cords and cables may not be used as a substitute for the fixed wiring of a structure.  Adjacent to the Moore Dryer, the employer did not ensure that a flexible cord was not used as a substitute for the fixed wiring of a structure, exposing employees to electrical and trip hazards. A flexible cord in use was affixed to electrical conduit and connected to a floor scale.
