29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At jobsite  6340 Jade Ave Derby KS, employees engaged in framing activities were exposed to electrical shock hazards in that the flexible cord protective layer was pulling away from connector.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At jobsite 6430 Jade Ave Derby KS, employees engaged in ground hand activities working below employees engaged in framing activities were exposed to struck-by hazards in that head protection was not utilized.  Joel Adalberto Fabian Robles was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1497519, citation number 1, item number 1 and was affirmed as a final order on 12-09-2020, with respect to a workplace located at 6414 S. Jade Ct. Derby KS.  29 CFR 1903.19(c)(2) abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At jobsite 6430 Jade Ave Derby KS , employee engaged in residential framing activities, walking and working on a 6:12 pitch roof, were exposed to fall hazards of  12 feet from the eave of the residence to the ground below in that fall protection was not utilized.  Joel Adalberto Fabian Robles was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1497519, citation number 1, item number 3 and was affirmed as a final order on 12-09-2020, with respect to a workplace located at 6414 S. Jade Ct. Derby KS.  29 CFR 1903.19(c)(2) abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At jobsite 6430 Jade Ave Derby KS, employees engaged in residential framing activities were exposed to falls from 12 feet, in that the aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof and was not secured to the building.  Joel Adalberto Fabian Robles was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1497519, citation number 1, item number 4/a and was affirmed as a final order on 12-09-2020, with respect to a workplace located at 6414 S. Jade Ct. Derby KS.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
