29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   (a) On or about 12/22/2020 and 04/08/2021 - Block sorting line,  employees were exposed to struck-by hazards when lockout/tagout was not utilized due to the written procedure not specifying that lockout/tagout was required for wood jams involving single pieces of wood/wood chips.

29 CFR  1910.147(c)(4)(ii)(C):The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them:  (a) On or about 01/06/2021 and 04/15/2021 - Lumber grading line, employees were exposed to struck-by hazards when the Block Sort Operator lockout/tagout procedure did not provide specific instructions for the order that locks were to be removed.

29 CFR  1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  (a) On or about 12/22/2020 and 04/08/2021 - Lumber sorting line, employees were exposed to struck-by hazards when employees had not been trained to apply five locks as required by the Block Sorter lockout/tagout procedure.  Employees had not been trained to use lockout/tagout for wood jams involving single pieces of wood/wood chips.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about 01/06/2021 and 04/15/2021 - Block sorting line, employees were exposed to caught-by hazards when the ingoing side of conveyor belts were not guarded where the conveyor belts went over rollers.  The belt pinch points  were located near the floor at the conveyor bringing lumber up to the block sorting line, at the operator stations, along the platform where the operators worked, and near the floor of the platform where employees cleaned debris.

29 CFR  1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements:  (a) On or about 01/06/2021 - 375 Coosa Road, Boaz, AL, employees were exposed to fall hazards when the concrete stairs from the production areas to the office and locker areas did not have handrails.
