29 CFR 1910.23(c)(7)(i): The employer did not ensure that ladders placed in locations such as passageways, doorways, or driveways where they can be displaced by other activities or traffic are secured to prevent accidental displacement:  (a) 319 Lockheed Street, Pueblo CO 81001: On or about January 6, 2021, and at times prior, employees were exposed to fall from height hazards due to ladders used to access higher levels not being secured to prevent displacement.  Extension ladders used to access the roofs of manufactured mobile homes were placed in high traffic areas and were not secured to prevent accidental displacement. 29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  (a)  319 Lockheed Street, Pueblo CO 81001: On or about January 8, 2021, and at times prior, employees in the roofing, HVAC, and electrical departments were exposed to fall hazards in excess of 12 feet while working on roofs of residential structures.  Employees were exposed while performing work on roofs of mobile homes at heights greater than 4 feet above a lower level.  Employees were not provided with fall protection systems to prevent exposure to fall hazards. (b) 319 Lockheed Street, Pueblo CO 81001: On or about January 8, 2021, the employer, Key Structures, LLC, did not provide OSHA with the requested OSHA 300, 300A, and 301 forms or equivalent within 4 business hours.  The employer did not provide OSHA form 301 or equivalent for an employee who was injured on January 6, 2021 due to a fall from a ladder.
