29 CFR 1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: (a)Kevin Hoppe Construction, Inc. at 2420 North Bakker Landing Avenue in Tea, SD 57064: On or about and at times prior to January 8, 2021, the controlling and exposing employer did not ensure a safety program was developed and implemented for the work environment to include regular inspections of the job-site, materials and equipment; conducted by a competent person. An employee and subcontractor employees were permitted to work in areas where fall hazards existed without the use of a fall protection system and were not provided ladders as a safe means of access/egress from the structure under construction. This condition exposed an employee and subcontractor employees to fall hazard of approximately 14 feet.

An employee and subcontractors used roof trusses and framing to climb down to the lower level of the building. This condition exposed one employee and subcontractors to fall hazards up to approximately 14 feet.
