At the workplace located at 504 Wilson Industrial Road, Maryville, MO: Employees were exposed to fall hazards. There were holes between the platform and concrete floor on Line 3 of the Riveting Line on the south east end of the Rivet Machine as the platform had not been moved all the way in place.

On or about January 7, 2021, a temporary employee received an amputation injury when the employee's hand was under the guard when the riveting machine was activated. b: The Stamco Rivet Machine on Line 2 was not adequately guarded as the riveter/assembly person working with the operator could reach through, under and around the guard.
