29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a)  On or about January 13, 2021, employees used pneumatic nail guns to install a roof on a new residential property without using safety glasses.  This exposed employees to a struck by hazard and an eye impalement injury from nails and/or other flying debris produced from roofing work.

29 CFR  1926.452(m)(2): Crawling boards shall be secured to the roof by ridge hooks or by means that meet equivalent criteria (e.g., strength and durability).  (a)  On or about January 13, 2021, employees were using crawling boards "chicken ladders" while installing a roof on a new residential property. The employer did not ensure the crawling board was properly secured to the roof, in-that the crawling board "chicken ladder(s)" were laid on the roof and supported by a toe guard/slide guard, exposing employees to a fall hazard of approximately 11 feet on the front side of the of the property.

29 CFR  1926.503(c)(3): 29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee.  (a)  On or about January 13, 2021, the employer did not ensure employees exposed to potential fall hazards had been properly trained to recognize and minimize the fall hazards associated with their work.  This is to include proper installation of anchor points and the proper selection of fall protection equipment.

The L.H. Krueger and Son, Inc. company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(11), which was contained in OSHA inspection number 1352044, citation number 1, item number 1 and was affirmed as a final order on January 2, 2019, with respect to a workplace located at 1345 Blazing Star Court in Oconomowoc, WI 53066.

This exposed employees to a fall hazard of approximately 11 feet on the front of the property and 20 feet on the back of the property.  The L.H.
