29 CFR  1910.332(b)(1): Practices addressed in this standard. Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.  Client delivery sites (e.g. 1138 Backbone Rd. in Tampico, IL 61283), bag/bulk delivery persons/truck drivers, feed truck/trailer top-mounted augers: on or about January 14, 2021, the employer failed to train personnel in practices to maintain safe clearances of at least 10 feet or more from the energized overhead lines when maneuvering feed augers during feed deliveries. Employees using ground controls on delivery equipment were exposed to direct contact with electric power distribution lines (120V to 7.8 kV) likely to result in electrocution. Employees did not know about safe clearances and practices to position vehicles and operate equipment at the required distances away from power lines.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.333(c)(3)(iii)(A): Any vehicle or mechanical equipment capable of having parts of its structure elevated near energized overhead lines shall be operated so that a clearance of 10 ft. (305 cm) is maintained. If the voltage is higher than 50kV, the clearance shall be increased 4 in. (10 cm) for every 10kV over that voltage.  Client delivery sites (e.g. 1138 Backbone Rd. in Tampico, IL 61283), bag/bulk delivery persons/truck drivers, feed truck/trailer top-mounted augers: on or about January 14, 2021, the employer failed to ensure that a clearance of at least 10 feet or more from the energized overhead lines was maintained when employees maneuvered feed augers. Employees using ground controls on delivery equipment were exposed to direct contact with electric power distribution lines (120V to 7.8 kV) likely to result in electrocution. Employees did not know about safe clearances and practices to position vehicles and operate equipment at the required distances away from power lines. 29 CFR 1910.333(c)(4)(ii): Where lack of illumination or an obstruction precludes observation of the work to be performed, employees may not perform tasks near exposed energized parts. Employees may not reach blindly into areas which may contain energized parts.  Client delivery site (1138 Backbone Rd. in Tampico, IL 61283), bag/bulk delivery persons/truck drivers, CEI Pacer feed trailer (serial No. AT65-30-2404) top-mounted auger: on or about January 14, 2021, the employer failed to ensure that adequate illumination was present when requiring employees to operate  equipment near energized overhead power lines.  Employees operating augers while making feed deliveries were working in the dark outdoors without any source of overhead illumination to identify and avoid overhead power lines.  An employee handling ground controls to maneuver the auger, and another employee nearby, were exposed to direct contact with about 7.8 kV electric power distribution lines likely to result in electrocution.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.  On or about January 14, 2021, the employer did not use and maintain OSHA Form 300 "Log of Work-Related Injuries and Illnesses" and OSHA Form 300A "Summary of Work-Related Injuries and Illnesses" for recordable injuries in the workplace. The Illinois Form 45 "Employer's First Report of Injury" was maintained by the employer, but this document is not a substitute replacement for OSHA Form 300 "Log of Work-Related Injuries and Illnesses", as it does not contain all required information.  There is no abatement documentation or certification required for this item.
