(a)  On the roof:   On or about 1/19/2021, eye and face protective equipment was not used when employees were using pneumatic nail guns.  Jenkins Home Improvement was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1167396, citation number 001, item number 03 and was affirmed as a final order on 7/31/17, with respect to a workplace located at 41 Pleasant View Drive, North Providence, RI 02904.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a. On the roof of the building On or about 1/19/2021, employees were not protected from falling while working on a roof approximately 7' 9 " in height.  Jenkins Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1343365 , citation number 002, item number 01 and was affirmed as a final order on 10/22/2018, with respect to a workplace located at 10 Log Road Smithfield, RI 02917.  Jenkins Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1167396 , citation number 002, item number 01 and was affirmed as a final order on 7/31/2017, with respect to a workplace located at 41 Pleasant View Drive North Providence, RI 02904

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a. At the worksite  On or about 1/19/2021, employees were exposed to fall hazards from utilizing an extension ladder to access the roof, without the ladder rails extending at least three feet above the upper landing of the roof.  Jenkins Home Improvement was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1167396, citation number 001, item number 04 and was affirmed as a final order on 7/31/17, with respect to a workplace located at 41 Pleasant View Drive North Providence, RI 02904.
