On or about March 16, 2021, the employer failed to implement and administer a hearing conservation for an employee operating the chop saw on the 1st Flr.  The employee was exposed to a dose of 202.1% during the 475 minute sampling period.  This exposure resulted in an 8-hour time-weighted average (TWA) of 95.3 decibels as measured on the A scale (dBA), exceeding the 85 dBA requirement of this part. Zero exposure was assumed for the time not sampled. The employee had not received baseline audiometric testing and had not participated in a training program as required by this part.  b.  On or about March 16, 2021, the employer failed to implement and administer a hearing conservation for an employee operating the carpentry machinery on the 3rd Flr.  The employee was exposed to a dose of 57.79% during the 460 minute sampling period.  This exposure resulted in an 8-hour time-weighted average (TWA) of 86 decibels as measured on the A scale (dBA), exceeding the 85 dBA requirement of this part. Zero exposure was assumed for the time not sampled. The employee had not received baseline audiometric testing and had not participated in a training program as required by this part.  c. On or about March 16, 2021, the employer failed to implement and administer a hearing conservation for an employee who stacks lumber.  The employee was exposed to a dose of 54.75% during the 208 minute sampling period.  This exposure resulted in an 8-hour time-weighted average (TWA) of 85.6 decibels as measured on the A scale (dBA), exceeding the 85 dBA requirement of this part.  Zero exposure was assumed for the time not sampled. The employee had not received baseline audiometric testing and had not participated in a training program as required by this part.  d. On or about March 16, 2021, the employer failed to implement and administer a hearing conservation for an employee operating carpentry machinery.  The employee was exposed to a dose of 59.12% during the 212 minute sampling period.  This exposure resulted in an 8-hour time-weighted average (TWA) of 86.2 decibels as measured on the A scale (dBA), exceeding the 85 dBA requirement of this part.  Zero exposure was assumed for the time not sampled. The employee had not received baseline audiometric testing and had not participated in a training program as required by this part.
