29 CFR  1910.157(g)(1): Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer shall also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.  a. On or about January 20, 2021, and at times prior, in the the woodshop, a Yale powered industrial forklift was in operation that had been found in need of repair. The forklift was found to have a missing seatbelt but was not taken out of service.

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  a. 29 CFR  1910.37(a)(3):Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  a. On or about and proceeding January 20, 2021, the front (North facing) exit  route was not free and unobstructed from a build-up of materials being processed, scrap wood and other items  which were stored/staged in the area in/around the front emergency exit door and obstructing entry/exit into and out of the area in the event of an emergency.
