29 CFR 1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about January 21, 2021, at 152 Azure Mist Way, Daytona Beach, Florida 32124, employees performing roofing work on a low roof of a two-story residential structure, were using a lanyard with harness and anchor which was extending beyond ground level, allowing the workers to free fall to ground level.

On or about January 21, 2021, employees were using a 24-foot Louisville extension ladder to access a lower roof improperly, as the ladder did not extend 3 feet above the eave, exposing employees to a fall hazard
