Stairs or a ramp must be provided where the exit route is not substantially level. a. On or about January 21, 2021, the employer did not ensure that the exit route was free and unobstructed at the shed storage building location of the work site when the exit door was blocked with the large iron claw-like part. b. On or about January 21, 2021, the employer did not ensure that the exit route was free and unobstructed at the shed storage building location of the work site when the exit door was blocked with wood pallets, palletized product and pallet jack.

The sling device was attached to the Toyota Forklift Truck, Model 8FGCU25, S/N 51008, Company Tag FL01 mast to move the 40 foot S4 steel beam bundle.
