29 CFR  1910.37(a)(3):  Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  a.  On or about January 21, 2021, the employer did not ensure that the exit route was free and unobstructed at the shed storage building location of the work site when the exit door was blocked with the large iron claw-like part.  b.  On or about January 21, 2021, the employer did not ensure that the exit route was free and unobstructed at the shed storage building location of the work site when the exit door was blocked with wood pallets, palletized product and pallet jack.

29 CFR  1910.157(c)(1):  The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  a. On or about January 21, 2021, the employer did not ensure that the fire extinguisher was readily accessible to employees without subjecting the employees to possible injury when attempting to access the fire extinguisher that was inaccessible with multiple parts/products and materials at the identified fire extinguisher location.  b. On or about January 21, 2021, the employer did not ensure that the fire extinguisher was readily accessible to employees without subjecting the employees to possible injury when attempting to access the fire extinguisher that was inaccessible with palletized product boxes at the identified fire extinguisher location.  c. On or about January 21, 2021, the employer did not ensure that the fire extinguisher was readily accessible to employees without subjecting the employees to possible injury when attempting to access the fire extinguisher that was inaccessible with palletized product boxes and wired shelf racks at the identified fire extinguisher location.  d. On or about January 21, 2021, the employer did not ensure that the fire extinguisher was readily accessible to employees without subjecting the employees to possible injury when attempting to access the fire extinguisher that was inaccessible with multiple stacked product boxes at the identified fire extinguisher location.

On or about January 13, 2021, the employer permitted the use of the rusted clevis grab hook and steel alloy chain to be used as a sling without the permanently affixed durable identification stating size, grade, rated capacity and reach. The sling device was attached to the Toyota Forklift Truck, Model 8FGCU25, S/N 51008, Company Tag FL01 mast to move the 40 foot S4 steel beam bundle.

29 CFR  1910.184(e)(2)(i):  Hooks, rings, oblong links, pear shaped links, welded or mechanical coupling links or other attachments shall have a rated capacity at least equal to that of the alloy steel chain with which they are used or the sling shall not be used in excess of the rated capacity of the weakest component.  a.  On or about January 13, 2021, the employer failed to verify that the hooks and oblong links were rated for the capacity or equal to the steel alloy chain that was used as a sling to move the 40 foot S4 beam bundles at the shed warehouse location.
