York VA Medical Center Hospital - Building 9: On or about 1/25/2021 and days prior, the employer exposed employees to a chemical hazard, the employer did not have a product identifier that was referenced with the appropriate safety data sheets for all-purpose joint compound and liquid nail construction adhesive while employees performed fabrication of new interior walls. Semper Tek, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(e)(1)(i), which was contained in OSHA inspection number 1115858, citation number 2, item number 1 and was affirmed as a final order on 04/11/2016, with respect to a workplace located at 800 Zorn Ave, Lousiville, KY.
