29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): (a) Jobsite - The employer did not ensure that all working levels of a tubular welded scaffold, approximately 80 feet long, 20 feet high and 5 feet wide, was not fully planked across the entire work levels exposing employees to fall hazards of approximately twenty (20) feet. Observed on or about 08 JAN 2021. GTO Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1366683, citation number 1, item number 1 and was affirmed as a final order on 04 OCT 2019, with respect to a workplace located at 3200 Napoli Way, Philadelphia, PA 19146.

(a) Jobsite - The employer did not ensure that tubular welded scaffolding, approximately 80 feet long, 20 feet high and 5 feet wide used onsite was equipped with guardrails or personal fall arrest systems when employees were working from the third working platform thereby exposing employees to fall hazards of approximately twenty (20) feet. Observed on or about 08 JAN 2021. GTO Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.451(g)(1), which was contained in OSHA inspection number 1366683, citation number 1, item number 2a and was affirmed as a final order on 04 OCT 2019, with respect to a workplace located at 3200 Napoli Way, Philadelphia, Pennsylvania 19146.
