29 CFR  1926.451(c)(2):Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  (a) Jobsite - The employer did not ensure that baseplates of a tubular welded scaffold, approximately 80 feet long, 20  feet high and 5 feet wide were placed completely on mud sills or other adequate firm foundation, thereby exposing employees to possible fall hazards and scaffold collapse. Observed on or about 08 JAN 2021.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Jobsite - The employer did not ensure that the attached portable ladder used to gain access to the third working level extended past three (3) feet past the working level exposing employees to fall hazards of approximately twenty (20) feet. Observed on or about 08 JAN 2021.

29 CFR  1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  (a) Jobsite - The employer did not ensure that all working levels of a tubular welded scaffold, approximately 80 feet long, 20  feet high and 5 feet wide, was not fully planked across the entire work levels exposing employees to fall hazards of approximately twenty (20) feet.  Observed on or about 08 JAN 2021.    GTO Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1366683,  citation number 1, item number 1 and was affirmed as a final order on 04 OCT 2019, with respect to a workplace located at 3200 Napoli Way, Philadelphia, PA 19146.

29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  (a) Jobsite - The employer did not ensure that tubular welded scaffolding, approximately 80 feet long, 20  feet high and 5 feet wide used onsite was equipped with guardrails or personal fall arrest systems when employees were working from the third working platform thereby exposing employees to fall hazards of approximately twenty (20) feet. Observed on or about 08 JAN 2021.  GTO Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.451(g)(1), which was contained in OSHA inspection number 1366683, citation number 1, item number 2a and was affirmed as a final order on 04 OCT 2019, with respect to a workplace located at 3200 Napoli Way, Philadelphia, Pennsylvania 19146.
