29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Faddis Concrete Products site located at 3515 Kings Highway, Downingtown, PA 19335:  The path to the eye wash station in "Area 4" of production was blocked by chemical containers and used as a storage. The eye wash station was not accessible for immediate use where employees work with corrosive materials, such as ProSpec BlendCrete.  Condition was noted on or about January 25, 2021.

29 CFR  1910.1053(d)(3)(iv): Where the most recent exposure monitoring indicated that employee exposures were above the PEL, the employer did not repeat such monitoring within three months of the most recent monitoring:  a) Faddis Concrete Products site located at 3515 Kings Highway, Downingtown, PA 19335:  The employer did not repeat respirable crystalline silica exposure monitoring within the required timeframe. Sampling was last conducted in 2017 and revealed an employee was exposed to an 8-hour TWA of 65 �g/m3 , which exceeds the PEL of 50 �g/m3. The condition was noted on or about January 25, 2021.

29 CFR  1910.1053(h)(1):The employer allowed dry sweeping or dry brushing where such activity could contribute to employee exposure to respirable crystalline silica and wet sweeping, HEPA-filtered vacuuming or other methods that minimize the likelihood of exposure were feasible:  a) Faddis Concrete Products site located at 3515 Kings Highway, Downingtown, PA 19335:  The employer permits dry sweeping of the plant floor, thus potentially contributing to employee exposure to respirable crystalline silica. Silica dust is generated and accumulates on the plant floor from production of concrete products. The condition was noted on or about January 25, 2021.
