29 CFR  1910.157(e)(1): The employer shall be responsible for the inspection, maintenance and testing of all portable fire extinguishers in the workplace:   On or about January 22, 2021, in the shop, employees engaged in welding activities were not provided with adequately inspected, maintained, and tested fire extinguishers in the following instances:  a.  Ansul Sentry, No. AY-251951, Model PK20; b.  Amerex, Model A417; and, c.  Kidde, No. F56239375.

29 CFR  1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substance:    On or about January 22, 2021, at this establishment, torching cutting activities performed on scrap, including a metal tank, were conducted without prior cleaning of the tank to ensure flammable materials were not present, inside the tank, exposing employees to deflagration.
