29 CFR 1926.451(f)(7): The scaffold was erected, moved, dismantled, or altered without the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration: (a) RSL Contractors, LTD at 1160 E. 18th Ave, Denver, CO 80218: On and before January 21, 2021 the exposing employer did not ensure the scaffold platforms had been inspected for visible defects by a competent person before use. Without the inspections employees were exposed to fall hazards of approximately 10 feet to over 60 feet in instances such as, but not limited to: 1. Metal scaffold platforms, used for exterior sheathing work, were resting on outrigger beams on the exterior of the building but were not safely secured to the outrigger beams to prevent the platforms from tipping, lateral movement, or falling off the beams. 2. A wooden scaffold platform, used for exterior sheathing work, was supported on each end by a scissors lift but was not secured in such a manner to prevent the platform from lateral movement or falling from the scissors lifts.
