29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail systems, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a)Advanced Roofing at 402 2nd Street, Rapid City, SD 57701: On or about and at times prior to January 27, 2021, the employer did not ensure employees were protected from fall hazards associated with a commercial roofing project. Employees were engaged in the installation of a PVC roofing system on a commercial building without a fall protection system. This condition exposed employees to a fall hazard of approximately 15 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing, and was not secured: (a)Advanced Roofing at 402 2nd Street, Rapid City, SD 57701: On or about and at times prior to January 27, 2021, the employer did not ensure employees were protected from fall hazards associated with accessing/egressing a flat commercial roof using an extension ladder with the side rails not extended at least three feet above the upper landing. Employees were utilizing a Little Giant extension ladder that did not extend three feet past the roof line. This condition exposed employees to an uncontrolled fall hazard of approximately 15 feet from the roof to the ground. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
