29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Alloyd #1 Straight Line Packaging Machine: On or about January 28, 2021, the employer did not ensure that the employee operating the Alloyd #1 machine was protected from moving parts at the point of use, by affixing a guard over an unused opening where blister cards are fed into the machine. The employee suffered a partial finger amputation while attempting to retrieve a card that fell into the unguarded unused opening.
