Orange extension cord used to energize the Raymond pallet jack was damaged in three locations, exposing the inner coated conductors. B. Black charging cord provided for the middle Raymond pallet jack had a crushed/split male end, exposing the inner components. C. Black charging cord provided for the right Raymond pallet jack had a split out jacket, exposing the inner coated conductors.
