29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) RM Construction 17683 Eclipse St., Broomfield, CO 80023: On and before February 4, 2021, the exposing employer did not ensure that employees were protected from injury, in that three employees were observed conducting framing operations on a new residential construction without the use of personal fall arrest systems. The employer did not protect employees against falls at heights over 10 feet. This condition exposes to employees to head injuries and broken bones.

29 CFR  1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178.     a) RM Construction 17683 Eclipse St., Broomfield, CO 80023: On and before February 4, 2021, the employer did not ensure that forklift operators were trained in the safe operation of powered industrial trucks, in that employees were working under suspended loads while working on the second level of a residential home. The employer did not protect employees from struck by and overhead hazards. This condition exposed employees to struck-by hazards.  Note:  The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) RM Construction 17683 Eclipse St., Broomfield, CO 80023: On and before February 4, 2021, the exposing employer did not ensure that employees were protected from injury, in that portable extension ladders, used to access the first level a residential home, had side rails extending 3 feet above the upper landing, and was secured. The employer did not protect employees against falls at heights over 10 feet. This condition exposes to employees to head injuries and broken bones.
