29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about  February 9, 2021, at the worksite located at W248 N5499 Executive Dr., Sussex, WI, the employer did not ensure adequate guarding was implemented on the # 1 Komo Xtreme CNC router machine. Elements of the machine including, but not limited to, the travel area of the material table, were not guarded, exposing workers to struck-by hazards.    (b) On or about  February 9, 2021, at the worksite located at W248 N5499 Executive Dr., Sussex, WI, the employer did not ensure adequate guarding was implemented on the # 3 Komo Xtreme CNC router machine. Elements of the machine including, but not limited to, the travel area of the material table, were not guarded, exposing workers to struck-by hazards.    (c) On or about  February 9, 2021, at the worksite located at W248 N5499 Executive Dr., Sussex, WI, the employer did not ensure adequate guarding was implemented on the # 4 Komo Xtreme CNC router machine. Elements of the machine including, but not limited to, the travel area of the material table, were not guarded, exposing workers to struck-by hazards.    (d) On or about  February 9, 2021, at the worksite located at W248 N5499 Executive Dr., Sussex, WI, the employer did not ensure adequate guarding was implemented on the # 1 MultiCam 5000 CNC router machine. Elements of the machine including, but not limited to, the support structure of the cutter head, were not guarded, exposing workers to struck-by hazards.    (e) On or about  February 9, 2021, at the worksite located at W248 N5499 Executive Dr., Sussex, WI, the employer did not ensure adequate guarding was implemented on the # 2 MultiCam 5000 CNC router machine. Elements of the machine including, but not limited to, the support structure of the cutter head, were not guarded, exposing workers to struck-by hazards.    (f) On or about  February 9, 2021, at the worksite located at W248 N5499 Executive Dr., Sussex, WI, the employer did not ensure adequate guarding was implemented on the # 3 MultiCam 5000 CNC router machine. (h) On or about  February 9, 2021, at the worksite located at W248 N5499 Executive Dr., Sussex, WI, the employer did not ensure adequate guarding was implemented on the # 5 MultiCam 5000 CNC router machine. Elements of the machine including, but not limited to, the support structure of the cutter head, were not guarded, exposing workers to struck-by hazards.  (i) On or about  February 9, 2021, at the worksite located at W248 N5499 Executive Dr., Sussex, WI, the employer did not ensure a Jet drill press machine was adequately guarded, thereby exposing workers to entanglement hazards.   (j) On or about  February 9, 2021, at the worksite located at W248 N5499 Executive Dr., Sussex, WI, the employer did not ensure a Delta drill press machine was adequately guarded, thereby exposing workers to entanglement hazards.

29 CFR  1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  (a) On or about  February 9, 2021, at the worksite located at W248 N5499 Executive Dr., Sussex, WI, the employer did not ensure the guard on the Kreg 5 Hole Pocket Drill was adequately secured. The guard, which consisted of a cover, was loose and was not fixed to the machine, exposing employees to laceration injuries.
