29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: WORKSHOP/BLENDING ROOM: The opening of the grate to the augur of the SL 100 Fitz mill G6 mill machine was partially unguarded and exposed the employees to cuts, abrasions, lacerations and amputation during the pouring of the product bag to the hopper for the grinding operation of the mill. On/or about January 28, 2021, the employee/injured blending operator had opened the 50 pound product bag and was feeding the product to the hopper at the mill. The product was processing through the mill and the employee/injured blending operator was pouring the product with his right hand while holding his left hand on the hopper. The employee right middle finger on the right hand was amputated. ABATED DURING THE INSPECTION.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. WORKSHOP: The employer had not reported the partial finger amputation to Federal OSHA on the injured/employee resulting from a partial guarded augur to the SL 100 Fitz mill G6 mill machine. ABATED DURING THE INSPECTION.
