Solution Room 1- Reactor and Holding Tank 1: On or about April 13th 2021, the employer did not adequately develop and utilize a procedure to control the thermal and pressure hazards created when the operator drained hot water from the 3000 gallon tank during maintenance operations.

Solution Room 1- Reactor and Holding Tank 1: On or about April 13th 2021, the employer did not provide adequate training to the operator on the knowledge and skills required for controlling the thermal and pressure energy released during the tank cleaning process.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment. Solution Room 1- Reactor and Holding Tank 1: On or about April 13th 2021, the employer did not select and have the operator use any type of overcoat or apron to protect the body from exposure to hot water, when draining 180 degree Fahrenheit water from a 3000 gallon tank. The operator was burned when the piping elbow rotated and hot water splashed onto the worker.
