29 CFR  1910.176(a):Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.  On or about February 11, 2021, in the Micro Irrigation Business Section of the warehouse, on isle #16 there was structural damage to the metal pillar (the metal pillar was cut through at the bottom on one side).  The structural integrity of the beam/building was unknown.  This condition exposed employees to  stuck by and/or caught between from possible building collapse.

29 CFR  1910.151(c):Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  On or about February 11, 2021, the eyewash station located next to loading dock #58 was not free from debris nor kept clean and in good working condition.

29 CFR  1910.178(l)(6):Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  On or about February 11, 2021 the employer did not certify that each operator had been trained and evaluated as required in paraph (l). The certification procedures for operating a sit down forklift was missing the date of the evaluation, the identity of the person(s) performing the training or evaluation, and if the operator had passed or failed.
