29 CFR  1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color:   (a) K&B Solutions Warehouse, front office area: On or about 02/16/2021, employees were not protected from potential smoke inhalation and/or other fire related hazards, in that exit signs above the only exit from the offices were damaged, inoperable, and not  illuminate and lighting was not available in the event of a fire or power loss.   (b)  K&B Solutions Warehouse, paint shop: On or about 02/16/2021, employees were not protected from potential smoke inhalation and other fire related hazards, in that exit signs in a room full of flammable liquids were not illuminated at the only exit, and lighting was not available in the event of a fire or power loss.   (c)  K&B Solutions Warehouse, warehouse entrance from the lobby/showroom: On or about 02/16/2021, employees were exposed to trip hazards or potential smoke inhalation, in that an exit sign above the entrance to the front of the building was not illuminated in the event of a fire or loss of power.

This condition exposed employees to a industrial vehicle struck-by hazards.

29 CFR  1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition:  (a) K&B Solutions Warehouse: On or about 02/16/2021, the employer did not ensure that the Mitsubishi (Model FGCZ5N) Forklift Truck which was in need of repair due to defective or non-operable horns, was not taken out of service until it had been restored to safe operating conditions. This condition exposed employees to a struck-by hazard.  (b) K&B Solutions Warehouse: On or about 02/16/2021, the employer did not ensure that the Mitsubishi EOP Order Picker Forklift Truck which was in need of repair due to defective or non-operable horns and strobe lights, was not taken out of service until it had been restored to safe operating conditions. This condition exposed employees to a struck-by hazard.

Employees operated a Mitsubishi fork truck (Model #:FGCZ5N) with a horn that was inoperable. The  powered industrial trucks was not inspected before being placed into service. This condition exposed employees to a struck-by hazards.   (b) K&B Solutions Warehouse: On or about 02/16/2021, the employer did not ensure that industrial trucks were examined before being placed in service. Employees operated a Mitsubishi EOP Order Picker Forklift with a horn that was inoperable and strobe light which was inoperable. The  powered industrial trucks was not inspected before being placed into service. This condition exposed employees to a struck-by hazards.

The casing of the extension cord used in the cabinet fabrication/customizing area was taped together as a temporary repair.
