29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) SHOP 3-Two metal receptacle boxes powered by a flexible cord and plug were "daisy chained", where the first metal receptacle box was plugged into a second metal receptacle box, exposing employees to electrical shock hazards. b) HOUSEKEEPING OFFICE- A 15A relocatable power tap was supplying power to a microwave (14A), a computer workstation and printer, exposing employees to electrical shock hazards and burns. c) HOUSEKEEPING OFFICE- A relocatable power tap was "daisy chained" and supplying power to an extension cord and second relocatable power tap, exposing employees to electrical shock hazards. d) LAUNDRY ROOM- A relocatable power tap was mounted to a wall and supplying power to the ProClean automatic detergent dispensing system, exposing employees to electrical shock hazards.
