29 CFR 1910.140(c)(16): The employer did not ensure that personal fall protection systems and their components are used exclusively for employee fall protection and not for any other purpose, such as hoisting equipment or materials.  MAINTENANCE SHOP: Employees performing scheduled maintenance on ventilation equipment are anchoring personal fall arrest systems to ventilation equipment not designed exclusively for fall protection, exposing them to fall hazards.

29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  FACILITY WIDE: Employees perform maintenance on HVAC and boiler equipment that require lock out tag out procedures. The employer had not established a program consisting of energy control procedures, employee training and inspections to ensure the startup or release of stored energy could not occur while servicing or maintenance, exposing employees to stored energy hazards.
