29 CFR 1910.119(e)(3)(i):The process hazard analysis did not address the hazards of the process: The employer failed to ensure that the 2016 Process Hazard Analysis addressed the hazard(s) of the loss of steam heat in the chlorine room. On or about February 16, 2021, a chlorine leak occurred when low external temperatures combined with the loss of steam boiler heat caused a pipe/lead washer/equipment malfunction.

29 CFR 1910.119(e)(6): The process hazard analysis was not revalidated at least every five years to assure its consistency with the current process: The employer failed to ensure that the 2016 Process Hazard Analysis adequately addressed the current process hazard of loss of power and temperature dropping below the safe operating temperatures. On or about February 16, 2021, a chlorine leak occurred when low external temperatures combined with the loss of steam boiler heat caused a pipe/lead washer/equipment malfunction.
