29 CFR 1910.28(b)(1)(i): 29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): (a) On or about February 15, 2021, an employee was exposed to a fall hazard while conducting service work on the Line 1 palletizer/stacker machine. This exposed an employee to a fall hazard greater than 4 feet from the lower level.

29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into area on a machine or piece of equipment's point of operation. (a) On or about February 15, 2021, an employee was exposed to a caught-in hazard when alternative means of protection during equipment servicing on Line 1 palletizer/stacker machine were not utilized. This exposed an employee to a caught-in hazard and amputation injury.

This exposed an employee to amputation injury.
