29 CFR  1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected:  a.) Line 3 Rademacher - On or about January 3, 2021, an authorized employee performed preventive maintenance on the line 3 Rademacher machine (order no. 7700283).  The employer failed to provide a periodic inspection that included a review between the inspector and each authorized employee and the employee's responsibility under the energy control procedure.  b.) Line 3 Rademacher - On or about February 17, 2021, an authorized employee performed the service task of dismantling the laminator guard on the line 3 Rademacher machine.  The employer failed to provide a periodic inspection that included a review between the inspector and each authorized employee and the employee's responsibility under the energy control procedure.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a.) Sanitation, Line 3 Rademacher  - On or about February 17, 2021, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices.   A sanitation employee sustained fractures to the wrist and forearm when the employee's right hand glove was pulled in-between the roller and bar of the conveyor.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a.) Line 3 Rademacher - On or about February 17, 2021,  an employee was exposed to machine hazards associated with moving parts and pinch-points while cleaning the L3 Rademacher machine, resulting in fractures to the right wrist and forearm.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
