29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Thermo Pac, LLC, 1609 Stone Ridge Drive, Stone Mountain, GA - Employees were exposed to amputation and caught-in hazards while engaged in changing the film of the Lane Machine 7007. The employer had not developed adequate procedures for processing employees to control the multiple energy sources to include, but not limited to thermal, electrical, and pneumatic energy. Furthermore, the employer failed to ensure that lock out/tag out procedures were utilized for the control of potentially hazardous energy, on or about 02/09/2021.

29 CFR 1910.147(c)(6)(i):The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: Thermo Pac, LLC, 1609 Stone Ridge Drive, Stone Mountain, GA (Throughout Facility) - Employees were exposed to amputation and caught-in hazards while engaged in changing the film of the Lane Machine 7007. Employees were exposed to sudden energization or start-up of multi-energy sources The employer did not conduct periodic inspections of the Lockout/Tagout procedures to assure that the procedures were being followed and corrections were made as needed to the multi-energy source machines throughout the workplace, on or about 02/09/2021.
