29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about February 23, 2021, and at times prior thereto, the employer did not develop and enforce the utilization of written procedures with specific steps for the control of potentially hazardous energy regarding machines and equipment such as, but not limited to, CNC machines, hydraulic presses, and bandsaws. Employees were exposed to caught-in and struck-by hazards from the release of stored energy.

29 CFR 1910.147(c)(7)(i):The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: On or about February 23, 2021, and at times prior thereto, the employer did not provide training on the energy control (lockout/tag out) program to ensure that all required elements of the program were understood by all authorized employees whose work operations were, or may be in an area where energy control procedures are utilized. Employees were exposed to caught-in and struck-by hazards from the release of stored energy.
