29 CFR 1926.605(b)(2): Unless employees can step safely to or from the wharf, float, barge, or river towboat, either a ramp, meeting the requirements of paragraph (b)(1) of this section, or a safe walkway, was not provided. (a) Port of Vancouver - Robishaw Flexifloat sectional barge crane access - On or about February 25, 2021 and at times prior thereto, employees were exposed to falling into the water while transiting from the pier to the barge crane (Manitowoc model 4100W serial 413419) in that the wooden decking had standing water and was slippery. The wooden platform was affected by the tide and river traffic, which also made the platform unstable.

(a) Robishaw Flexifloat Sectional Barge - Fuel storage room - On or about February 25, 2021 and at times prior thereto, employees were exposed to fire and smoke inhalation in that sodium hypochlorite was stored in the fuel storage room where numerous flammables are located to include, but not limited to: diesel, gasoline, motor oil, automatic transmission fluid, aerosol paints etc.

(a) Robishaw Flexifloat Sectional Barge- Fuel storage room - On or about February 25, 2021 and at times prior thereto, employees were exposed to fire and smoke inhalation in that there was not a 3' wide clear aisle to egress from the fuel storage room in the event of an emergency.

(a) DB 4100 derrick barge (Robishaw Flexifloat) - Lunch Room - On or about February 25, 2021 and at times prior thereto, employees were exposed to electrical shock, fire and smoke inhalations from the improper installation and usage of a Bilkin relocatable power tap (RPT) 15 Amps in that the Magic Chef Microwave 13.5 Amps (model: MCM990ST, serial no: 0629890800454) UL Listed 54NJ and the Toastmaster Toaster 8.5 amps (model T2020WT) UL Listed 485F were both connected to the Bilkin RPT.
